肌萎缩侧索硬化症患者非锥体束的磁共振扩散张量成像研究

- 及精神症状相一致。对已故ALS患者进行的尸检结果显示其丘脑发生了变性[11]。另外,在正电子发射计算机断层扫描(PET)研究[12]中,口语流畅性损害的ALS患者丘脑前核和复合体的局部脑血流(rCBF)反应减弱。家族性ALS患者的单光子发射断层扫描(SPECT)显示其双侧丘脑的血流灌注不足[13]。这均与本研究中ALS患者丘脑FA值的减低相一致。
本研究中健康对照组、病例组胼胝体膝部处FA值差异无统计学意义,这可能与该处纤维束密集性差、健康人该区域水分子各向异性较低有关。另外,两组间各ROI处MD的差异亦无统计学意义,这可能表明上述区域虽有损害,但细胞密度并未发生改变[14]。这是由于伴随的胶质细胞增生填充了由于纤维、细胞变性缺失而扩大的细胞外间隙,使得细胞密度保留,水分子的扩散特性未发生明显改变。由此可见,与MD比较,FA是脑组织微观结构损害更为敏感的指标。
综上所述,除中枢运动传导系统外,ALS患者脑部非运动区及锥体束以外的白质亦受累。Sage等[15]的研究亦表明,ALS的神经元变性不仅局限于锥体束,ALS更有可能是一种以运动系统损害为主的多系统变性疾病。尚需进行更深入的研究以阐明ALS的发病机制,以期早日找到真正切实有效的治疗ALS的方法。
